Yes! The information you provide in the online survey is anonymous. Unless you provide your name and contact information for a follow-up request, this survey will remain anonymous.

This survey will be filled out online. If you do not have access to a computer or the internet, a paper copy can be provided. You can access the online survey via a QR Code flier posted throughout the building. You can also access the survey online using this website.

We will have paper copies of the survey available for those who do not have access to the internet or a computer. These will be provided at our reception desks in both our clinic and our EHS department. Please complete the survey and drop it in the box provided in our lobby.

Anyone who utilizes our services including cancer screening, CONNECT referrals, subdivision review, disease investigation/follow-up, travel consultation, immunizations, MAP, Ryan White, tobacco use/prevention, wellness classes, worksite wellness, home visiting, WIC, onsite visits for establishments, those seeking permits, etc. Also, if you are accessing information online about public health in Gallatin County, you are a customer!

The survey will have space to provide your name and contact information if you feel you would like someone from our department to follow up with you. If you would rather reach out to us, the contact information for the department you interacted with is listed in the survey once you indicate you would like a follow-up.

Currently the survey is also available in Spanish. From the first page, click the flag icon located in the top right-hand corner of the survey and toggle from English to Spanish. We also have Spanish hard copies of the survey available at our reception desk.
